To receive Star Assembly, you must submit all of these on time and also achieve your membership quota by 6/30.

*** Use this form (X) only if instructed to do so by the Master. It is only intended for cases where the FC cannot update the list in Member Management (MM). (This will never happen if the officer list is updated in a timely manner.) Otherwise update in MM Assembly Administration and generate a PDF copy for the Master in the Print Center. Using MM is much easier than using the paper form.

Tax return	990	X	Most Assemblies will use the 990-N in the link. Due 5/15 for calendar year or 11/15 for fraternal year. Use form 990-EZ if you do not qualify for the 990-N.
RSVP	2863	X	Submit for every $500 given to a seminarian to get a $100 refund from Supreme up to a max of $400.
